3/8ths drive ratchet , extension /s  short socket from in front of starter , 5/8ths wrench from underneath . if installing a replacement starter , get one for a newer truck , like ask for one for a 1993 it will be a mini ... carefull the heavy wire is right off the "HOT" side of the battery ....... but you knew that ...

I just replaced the starter on my 78 a few weeks ago and this is how i did it. Got the socket on the nut from below and then went up top with extensions and a ujoint to undo it. My top one was a stud if I remember correctly.

I know on my small block transmission with a big block on the front of it (adapter plate), you can use one of those ratcheting closed end wrenches on the top bolt of the starter. Maybe because of my set up its different?
